A Slope Mod is a modification that effects the angle of a draggable transit network slope. In a game without a slope mod, these angles can be quite steep, but do change between networks (the slope between a Highway and Street differ).
What a slope mod does is it reduces the angle of a network's slope, creating a more gradual angle, making it look much more realistic. Several slope mods exist, some more gradual than others. A "standard" slope mod is usually about half the angle of the default slope angles, but others could be more strict or forgiving.
